🔵جزئیات تکمیل فرم پایش سلامت دانشجویان ورودی ۱۴۰۳ دانشگاه آزاد
🔹فرم پایش سلامت یک پرسش‌نامه است که برای ارزیابی وضعیت کلی سلامت دانشجویان استفاده می‌شود. این فرم شامل سوالاتی در مورد وضعیت جسمی، روانی و عاطفی می‌باشد.
🔹کلیه دانشجویان ورودی ۱۴۰۳ برای دسترسی مجدد به سامانه آموزشیار، می‌بایست نسبت به تکمیل پرسش‌نامه پایش سلامت در سامانه دانشجویار اقدام نمایند.
🔹برای دسترسی به پرسش‌نامه، بعد از ثبت‌نام و ورود به سامانه دانشجویار، بر روی گزینه "دکتر من" کلیک کنید و نسبت به تکمیل فرم پایش سلامت اقدام کنید.
⚠️نکته: اگر قبلا نسبت به تکمیل فرم پایش سلامت در سامانه بافر اقدام کرده‌اید، نیازی به تکمیل مجدد این فرم در سامانه دانشجویار نیست. @iau_fani
