❇️ Chance Fracture (flexion-distraction injury)

Flexion-distraction injuries result from pulling forces that lead to damage in the posterior and middle portions of the spine. These injuries often feature fractures through the posterior bones, and it's common for the posterior ligamentous complex to also be affected. Chance fractures are a well-known example caused by flexion-distraction forces. Such an injury can happen when the upper body's weight shifts forward while the torso remains anchored, creating tension across the spine. In these cases, the vertebral body is usually affected by flexion forces, whereas the posterior spinal structures are damaged by distraction forces.

🎯Treatment:

Chance fractures can be treated conservatively or surgically. In patients with purely bony injuries and no neurological deficits, non-surgical management with a cast or TLSO is usually effective, allowing natural healing. Neurological symptoms or ligamentous damage, however, are indications for surgeryβ€”most commonly long-segment posterior fixation with pedicle screws, with or without interbody fusion. Spinal decompression may be needed in some cases.
Fracture reduction is typically done on a Risser table via hyperextension, followed by a custom plaster or fiberglass orthosis worn for 8–12 weeks. Compliance can be challenging due to discomfort, but union rates are high. Rehabilitation is crucial for regaining strength and mobility.
Obese or large patients may not tolerate conservative treatment and are better suited for surgery. Minimally invasive pedicle screw techniques have reduced the risks associated with open procedures.
